The architecture that holds the face

The cheeks and midface are the structural foundation of the entire face. The fat pads that sit across the cheekbones, the bone beneath them, and the soft tissue that drapes between them give the face its youthful proportion, its lift, and much of the architecture that the rest of the face hangs from.

When this support thins, the change is visible everywhere. The lower face begins to soften, the lines around the mouth deepen, the under-eye reads hollower, and the jawline begins to blur. Many of what patients notice as separate concerns are in fact the same change, expressed in different places, and meeting it well begins here.

How the midface quietly thins

The midface begins to thin gradually in the mid-thirties for many patients and continues steadily across the decades that follow. Often the change is invisible to the patient in any given year and unmistakable in photographs taken five or ten years apart. The cheekbone appears less prominent, the under-eye begins to read hollower, and the nasolabial fold and marionette lines begin to deepen as the support above them thins.

The arc steepens through the years surrounding menopause and accelerates with significant weight loss or chronic illness. Patients who have maintained stable weight and supported their skin steadily tend to enter their fifties with more of the midface architecture intact than those who have not.
