Where the smile draws a line

The nasolabial fold is the line that runs from the side of the nose to the corner of the mouth, drawn into the face by years of smiling and by the gradual thinning of the midface above. It is among the most universal signs of facial aging, and one of the concerns patients most commonly mention when they begin to consider treatment.

What makes this concern particular is that the fold itself is often not where the work belongs. The cheek above has thinned, the soft tissue has descended, and the crease has deepened as a consequence. The most honest answer is therefore often a careful restoration of the support above, rather than filling the line directly.

How a crease becomes a fold

A subtle nasolabial fold is present from young adulthood for most faces, as a natural seam between cheek and upper lip. It begins to deepen meaningfully in the mid-thirties for many patients, as the support above thins and the soft tissue begins its long descent. By the forties and fifties the fold often reads more deeply, even at rest.

The arc is responsive to care. Faces whose midface has been steadily supported across the years often enter the fifties with the fold meaningfully more restrained than those whose midface has thinned more substantially. Earlier work, when the change is still gentle, is gentler work, and gentler work returns more.
