The breadth the upper face once held

The temples are the broad, gently convex regions of soft tissue that sit between the outer brow and the hairline, the area patients most often think of as the side of the upper face. In youth they read full, contributing to the breadth of the upper face and the lift of the brow's outer edge.

As the fat pad of the temple thins steadily across the decades, the area softens into a hollow, the outer brow descends along with it, and the upper face begins to read narrower than the patient remembers it being. The change is gentle in any single year and unmistakable across a decade, often visible to the camera and to anyone looking from the side before the mirror has caught up.

Temple hollowing tends to arrive in the mid-forties for many patients and to deepen across the fifties and sixties. The arc is gradual, and the change is among the less noticed of the long aging shifts, partly because the temple is not where most patients are looking when they study their faces.

The arc steepens with significant weight loss, with the years immediately surrounding menopause, and with chronic stress or illness. Patients who have maintained stable weight and steady self-care tend to enter temple hollowing later and less visibly than those who have not.
