An under-eye roll is a small ridge that appears just below the lower lash line when you smile, as a band of the muscle that rings the eye bunches up. Some find it makes the eye look smaller or more closed when smiling, or adds to a tired appearance beneath the eye.
It is a matter of muscle rather than volume or skin, which is why a very small, careful dose of relaxer can soften it. It is distinct from a hollow beneath the eye, which is a question of lost volume and met quite differently.
Treatment suits the patient bothered by a clear ridge beneath the eye when smiling, whose lower lid is otherwise healthy and well supported. It is at its best as a subtle softening, leaving the smile intact.
The roll is made by the lower band of the orbicularis oculi, the ring of muscle that closes the eye. In some faces this band is naturally fuller or more active, and it bunches visibly as the cheek rises in a smile. It is largely a matter of anatomy, present from early adulthood, rather than something that arrives with age.
It becomes more noticeable as the skin beneath the eye thins over the years, and in those who have had filler placed beneath the eye, which can make an existing roll more apparent.
