The double chin is the pad of fullness beneath the chin that softens the line of the jaw and the angle where the lower face meets the neck. The medical term is submental fullness, and it is one of the few cosmetic concerns that responds to a treatment with a definite ending.
Most double chins are not weight-related. They are anatomical: small pockets of fat that sit beneath the chin and persist regardless of overall body weight or fitness. Many patients with double chins are otherwise lean, and the persistence of the submental pad despite their best efforts is what brings them in.
Treatment with Kybella suits patients with a defined submental pad, particularly those who have tried weight loss and exercise without success in addressing it. Candidates should be at or near their stable weight, in good general health, and prepared for several days of swelling after each session.

Genetics is the principal cause. The submental fat pad is largely inherited, and patients who carry it often have parents or siblings who do as well. The pad becomes more visible with the decades as the skin around it loses elasticity and the neckline grows less defined, but the pad itself is largely set at birth.
Weight gain can enlarge the pad. Significant weight loss can sometimes reduce it but rarely eliminates it. Aging adds to the appearance because the loss of jaw definition above the pad makes the pad more prominent by contrast.

The double chin can be present at any age. Some patients have carried it since their teens; others develop it in the forties as the skin around it loses elasticity and the jawline grows softer. The arc is less age-driven than most cosmetic concerns, and the right time to treat is usually the time when the patient is bothered by it.
Once treated, the result is largely permanent. Fat cells reduced by Kybella do not return, and the underlying anatomy is genuinely changed. Significant weight gain can re-enlarge the surrounding fat, but the original treated pad does not rebuild itself.
