Concerns We Treat

Under-Eye Hollows

A hollow groove beneath the eye that casts a shadow and lends a tired, drawn look even when rested.

What it is

An under-eye hollow is a groove that forms where the lower lid meets the cheek, deep enough to cast a shadow that reads as a dark circle or a tired, drawn look. It is a question of lost volume and structure rather than pigment, which is why brightening creams do little for it.

It is met, when the area suits it, with a small and careful placement of hyaluronic acid filler to soften the shadow. It is one of the most delicate treatments on the face, and one of the easiest to overdo, so it is approached with particular restraint.

Why Patients Seek Treatment

Treatment suits the patient with a genuine hollow and good skin quality, whose tear trough casts a shadow they would like softened, and who understands the work is subtle and shared with the cheek above. It is at its best as a quiet reduction of the shadow, not an erasure.

UNderstanding the science

What causes it

The hollow deepens as the fat that once cushioned the under-eye thins and descends, and as the ligament that tethers the lid to the cheek becomes more defined against the loss around it. The thin skin of the area, and the small vessels close beneath it, deepen the shadow further.

Some carry a degree of hollowing from youth, a matter of inherited structure. For others it arrives and deepens with the years, often alongside a flattening of the cheek above.

Common Signs

You may notice:

  • A shadowed groove or dark circle beneath the eye that persists even when rested
  • A tired or drawn look that makeup struggles to cover
  • A hollow that deepens as the cheek above loses its fullness
  • An area that looks worse in overhead or harsh light

Why it changes over time

Under-eye hollowing tends to begin in the thirties and deepen through the decades that follow, as the fat beneath the eye thins and the cheek above descends. Because the change is tied to the volume of the whole midface, treating the cheek often improves the under-eye in turn, which is why the area is read together with its neighbor rather than alone.
