What it is

Jowls are the soft folds of skin that descend along the lower face as the structures beneath the jaw begin to give. The clean angle that once separated cheek from neck softens, and the line of the jaw blurs. The change is one of architecture rather than surface, and the surface alone tells very little of the story.

What causes it

The principal causes are the long, intertwined forces of facial aging. The fat pads of the cheek thin and slide downward across the decades, removing the lift that once held the jawline taut. The bone of the jaw resorbs subtly, removing the foundation against which the soft tissue was draped. Skin loses elasticity and can no longer recover from gravity the way it once could. The result is a face that reads softer at the edges, and a jawline less defined than it was.

Hormonal shifts, particularly the changes that arrive with menopause, accelerate the loss. Significant weight loss can dramatize the appearance of jowls by removing the underlying fullness that helped keep them from forming. Genetics decides how readily a given face shows the change, and at what age.

Common Signs

You may notice:

  • A softening of the line of the jaw, particularly in profile, where it once read more sharply
  • Small pads of skin descending along either side of the chin, the visible front of the jowl
  • A general blurring of the angle where the lower face meets the neck
  • Photographs that catch the change more clearly than the mirror, especially in side lighting

Why it changes over time

Jowling tends to arrive in the late forties and to deepen across the fifties and sixties. The arc is gradual, and almost no one notices the change in any single year. Across a decade, however, the difference is often clearly visible to the patient herself, and clearly visible to the camera much earlier than to the mirror.

The arc steepens through the years immediately surrounding menopause, when estrogen-driven collagen production falls more sharply. It accelerates with significant weight loss, prolonged illness, and chronic sun exposure. Patients who have steadily maintained their skin's resilience tend to enter jowling later and more gently than those who have not.
